Consent for Treatment
For child/adolescent clients, FVC requires a signed ‘Consent for Treatment’ from BOTH PARENTS REGARDLESS of marital status. You must bring two signed consent forms with you on the day of intake, or you will not be seen for your scheduled appointment.

If you are not in contact with your child’s other parent, consent for treatment can be handled through a lawyer, and/or the stand-alone Consent for Treatment form can be mailed/faxed separately to the office prior to your appointment.
